Employee's arm fractured when caught in chain drive

At approximately 7:00 a.m. on January 15, 2002 Employee #1, a worker of Diamond Forest Products Inc., was repairing a drive-motor chain that was dislodged from its sprocket. While he was making repairs to the chain, the equipment became re-energized, the employee's sleeve became entangled in the drive motor chain. The employee was hospitalized with a fractured arm.
