On September 26, 2001, a fully-loaded 6,000-gallon water truck of Cmx Constructor, Inc. fell 20 ft into an excavation off the vertically cut bank after the bank collapsed under the weight of the truck, with Employee #1, a driver, in the cab. The employee suffered fractures and was hospitalized
