Employee Falls During Catwalk Installation

On October 15, 2001, Employee #1 and several coworkers were installing a catwalk. The employees installed three sheets of decking on top of the catwalk support beams 16 feet, 8 inches from the ground floor. The catwalk support beams were placed in position without being fastened. Employees stood on the support beams during the decking installation. While standing at the leading edge, Employee #1 had one leg on one support and the other leg on the other support. Suddenly, the supports moved apart, causing Employee #1 to fall to the concrete, killing him. Employee #1 did not use any fall protection.
