One employee killed, second injured when goose neck fell

On October 19, 2001, Employee #1 and Employee #2 were working on a Talbert 40-ton crane, Model T3DW40HRG-1-1T, when the goose neck dislodged and fell. The goose neck was not supported as required by the manufacturer and there were no written plans as to the steps to be taken to perform maintenance on this piece of equipment. The goose neck fell, killing Employee #1 and severely injuring Employee #2, who was hospitalized.
