Employee #1 died from mechancial asphxysia

On May 5, 2001, Employee #1 used a bumper jack instead of a service jack to raise a vehicle, did not use jack stands or other positive means to prevent the raised vehicle from falling, and lay down to work under the raised vehicle. The vehicle shifted or the bumper jack slipped, causing the vehicle to fall on Employee #1. He died from mechanical asphyxia.
