Employee killed when neck broken by falling ice block

On September 19, 2001, Employee #1 and two other Sylvest Farms' employees were assigned to work in the big ice house, one of two at the plant. At approximately 2:45 p.m., a coworker, using a pick ax, was chipping ice from the top of an ice pile, approximately 7 feet above the floor, when a large chunk or chunks of ice broke loose and fell.
