Employee injured when scissor lift fell off trailer

On October 18, 2001, Employee #1 operated the scissor lift from its platform, 48 inches above the floor, and rolled it off the side edge of the trailer, which was approximately 38 inches above the pavement. The Sky Jack III, Model Number 4832 elevating work platform, 48 inches wide and 60 inches long, was being off-loaded from its parallel position on the trailer. The trailer was 102 inches wide and 51 feet long, of which the latter 15 feet was the mechanical ramp used to descend the equipment for delivery at the site. The employer did not have documentation reflecting a training outline nor was practical training provided to employees regarding the safe unloading procedures of the equipment by lowering their control panels and walking along-side the lifts as they were maneuvered off their transportation vehicles. Employee #1 suffered fractured bones in the left pelvis, right wrist and face, along with lacerations and soft tissue contusions as a result of falling approximately eight feet when the scissor lift he was operating rolled off the edge of its transportation trailer and onto its side on the adjacent concrete sidewalk. Employee #1 was hospitalized.
