Investigation Summary

Investigation Nr: 202330338
Event: 07/12/2001
Crane boom damaged in tip over

Employees were pouring an abutment for an overpass when the crane that they were using tipped over. The crane operator was picking a wooden bulkhead weighing approximately 100 lb that was positioned in the center of the abutment. The pick was made and the boom swung around from west to east. When the boom reached the point at which it was perpendicular to the crane, the weight of the extended boom was too great for the crane, without having the outriggers extended on that side, causing the crane to tip over. There was approximately 65 feet of boom extended. There were no serious injuries to employees, however, there was damage sustained to the boom of the crane.
