Employee suffers concussion in fall from plank

On June 26, 2001, Employee #1 was working in the attic space above the classroom area of an elementary school undergoing renovations. He was standing on a 2 by 12 wooden plank that was serving as a work platform when he slipped and fell approximately 13 ft onto the tile floor. Employee #1 suffered a concussion, for which he was hospitalized. Subsequent investigation revealed that no form of fall protection was available or in use, Employee #1 had no fall protection training, and no fall protection training certification had been prepared.
