Event: 06/20/2001
Employee killed in fall from top of powered industrial truck

On June 20, 2001, Employee #1 had climbed on top of the overhead guard of a powered industrial truck and slipped down between the mast and the overhead guard area. Employee #1 was found unconscious by another employee with his head caught between a horizontal member of the mast assembly and the front horizontal corner of the overhead guard. Employee #1 was hanging by his head with his feet off of the ground. Employee #1 received a concussion and was killed as a result of the fall. Employee #1 had been operating powered industrial trucks for the company since April of 2001. No records showed that Employee #1 had received training in the operation of powered industrial trucks.
