Investigation Summary
On October 5, 2001, Employee #1, a parts stacker, was asked to remove parts from a conveyor inside the 2000-ton press, while the operator left to retrieve some sheet metal to weld to the conveyor and stop the parts from falling off. Employee #1 was crushed and killed by the transfer bar, when the operator returned and started the press inch mode.
