Employee killed when crushed by falling forklift

On September 20, 2001, Employee #1 was lying on the ground, tightening a bolt underneath a forklift. A coworker jacked the forklift up prior to removing the oil pan. The coworker removed the oil pan and then released the jack, causing the forklift to descend and crush Employee #1's face. Employee #1 was killed.
