Employees suffer smoke inhalation when extinguishing fire

On August 2, 2001, Employees #1, #2, and #3 attempted to extinguish a fire caused by an RPS sheet that was pulled back into a 400 degree Celsius oven. The RPS sheet was pulled back into the oven when the material clamps on a headliner molding machine malfunctioned. The 8-ft by 4-ft and 8-in. RPS sheet fell from the clamps on to the ovens heater. The 1/8-in. thick sheet caught on fire inside the molding machine oven. Employees #1, #2, and #3 used approximately twelve 16-pound fire extinguishers to extinguish the fire. All three employees were transported, where they were treated for smoke inhalation.
