Employee dies when run over and crushed by truck

A crew was installing a running track at a school. The rubberized running track was outside the perimeter of the football field where stands existed. There were fences along the sides and end. A full size pickup truck was used to haul fifty pound bags of rubber material to the area the crew was working. The pickup was loaded in the back so that the rear bumper was less than a foot from the ground. The bags were stacked in the bed of the pickup past the roof height. The truck was equipped with side view mirrors but no backup alarm. No one directed the driver while he backed the truck down the track towards the crew at the far end of a straight away. The driver had not done this particular job before and had only obtained a learner's permit that required a qualified driver to ride with him which had not occurred. The driver followed the same routine that had been done before and was backing slowly down the middle of the track when he struck Employee #1. Employee #1 was overtaken by the truck as they were traveling the same direction. He was struck and folded up under the bumper and pinned. The driver stopped when he felt a bump. Employee #1 was removed and only showed signs of arm and leg lacerations. He died three weeks later in a hospital.
