Investigation Summary

At about 4:55 p.m. on July 17, 2002, Employee #1 finished a 15-20 minute break when he asked his fellow workers for assistance to "roll" a coke cooler by manually turning a wheel. Approximately 15 seconds later, as the employee walked away in the direction of the wheel, he collapsed and fell to the ground. CPR was administered on-site by a company employee trained in first-aid but they could not get a response. An emergency response team arrived 10-15 minutes later. Employee #1 was taken to a local hospital where he was later pronounced dead as a result of a massive heart attack.
