Employee Drowned

At approximately 3:30 p.m. on April 26, 2002, two employees had just docked the M/V 008 and were preparing the vessel for the evening. They had tied off the bow of the vessel, leaving the engine running so the vessel would not move while attempting to tie off the stern. The coworker had disembarked the vessel and was handed the power cord for the sump. The coworker then turned away from the vessel to plug it in. When the coworker turned back toward the vessel, Employee #1 was nowhere to be found. The coworker then went back onto the vessel to turn off the engine and look for Employee #1, but he could not be found. The coworker then got on the Public Address System to announce that there was a man overboard. The coworker did find Employee #1's work vest hanging on the valve of the gas cylinders located on the deck. At this point an alarm was sounded on the Mississippi River to alert passing vessels that someone was in the water. Other workboats came into the dock area with the crews using hook poles to check the bottom. Approximately 30 minutes later Employee #1 was found in water behind the M/V 008 in approximately 20 ft of water. An autopsy of Employee #1 was conducted by the Coroner's Office. The Coroner's findings listed the cause of death as asphyxia by drowning and coronary arteriosclerosis (stroke). This would indicate that after the coworker turned away to plug in the sump pump, Employee #1 suffered a stroke that made him disoriented. This probably resulted in Employee #1 removing his work vest and walking to the back of the vessel where he fell into the river. If the Employee #1 went around the outside of the control cab or through it to get to the aft of the vessel, there were numerous tripping hazards, such as the door lip or the lip around the edge of the deck, that could have aided in the employee falling overboard, and due to the stroke, drowned.
