Investigation Summary
On February 20, 2002, Employee #1 was operating a 14-Ton Grove hydraulic crane, RT58, Serial Number 19793 to move a 165 lb valve. Because of the narrow width of the dock, the crane's outriggers were not extended and reduced the stability of the vehicle. The valve was rigged and boom of the crane positioned between the one and two o'clock position. The crane lifted the load and proceeded from the two o'clock position in a clockwise direction. As he began to move the load toward the 6 o'clock position, the crane became unstable and tipped over at the 3 o'clock position. The crane fell into the river. Employee #1 attempted to escape the cab of the crane, but struck the back of his head upon the vehicle. The impact caused him to suffer a basilar skull fracture. He died from the traumatic head injury. Upon the retrieval of the crane from the river, the factors that contributed to the crane becoming overloaded were the mid and fly sections of the telescoping booms were not equally extended, and the tires were not inflated to the manufacturer's recommended pressures and the rear axle oscillation lockout was not working.
