Investigation Summary
On March 2, 2002, Employee #1 was vacuuming and cleaning the flour silo deadheads. Employee #1 was on the top level of the flour silo, standing on a metal catwalk with guardrails. The top guardrail was 48-inches from the catwalk base, the midrail was 24-inches from the base. The distance from the top guardrail to the ground was approximately 55-feet. No equipment was involved. Employee #1 suffered a broken hip and several broken or ruptured vertebra discs in the fall and was hospitalized.
