Employee Falls From Roof Not Using Fall Protection

On January 16, 2002, the crew arrived at the job site in the early afternoon. They decided to do preparatory work so that they could return to the site the next day and concentrate on installing shingles. Two workers were on the roof staging materials, setting up equipment etc. Employee #1 installed ice and water shield on the higher roof section. While installing the shield, he fell 13 feet 6 inches from the roof to the ground, fracturing his arm and ribs. The eave of the lower roof was measured at 10 feet above the ground. The roof pitch was 5 feet and 12 feet. No fall protection equipment was in use at the time. Employee #1 was hospitalized to treat his arm and ribs.
