Employee Suffers Amputation Using Unguarded Press

Employee #1 was operating a press and placed his hand in the point of operation to align the pieces to be pressed. The press was unguarded. The company training program includes a Press and Bender Machines Safety Checklist that, among other items, states, "Be careful not to place your fingers under the dies. However, Employee #1 had never been trained. Employee #1 suffered the amputation of fingers of his right hand. Employee #1 was hospitalized.
