Investigation Summary
Employee #1 and several other employees had removed the tires, axles and tongues from modular units that were being installed as spectator boxes at the Deer Valley Winter Venue. Approximately 10 to 20 modular tires had been strung together and Employee #1 was attempting to load the string of tires onto a trailer using a Lull 844C extended boom forklift. The employee had the forklift positioned on the side of the trailer. The trailer was facing down a hill and the forklift was at a right angle to the trailer, on a downhill slope. As the employee lifted the load, the tires began to swing and the forklift tipped over. The employee exited the cab of the forklift, and was struck by the extended boom. Employee #1 suffered a head laceration and a broken leg. Employee #1 did not appear to be trained on this type of forklift. The employee did not take into account that the forklift was on a slope prior to attempting to lift the load. In addition, he was not wearing a seat belt or his hardhat.
