Electric Shock - Direct Contact with Energized Parts

Employee #1 was cutting down (shortening) deenergized lines in the low-voltage (120/208-volt) side of a transformer so that the lines could be pulled out of the transformer. Either he lost his balance and went forward into the terminal lug, or he had his head in the transformer box and turned his head, contacting the terminal lug. Either way, he was electrocuted.
