Investigation Summary

Investigation Nr: 200011526
Event: 07/19/2002
Employee Killed When Caught in Plastic Roll Winder

On July 19, 2002, Employee #1 was putting 36 in. wide plastic rolls into a Future Machines winder that had an 84 in. wide feed plate. He apparently placed his left arm into the machine when it got caught, pulling Employee #1 into the unit. A coworker heard his screams and went to his aid. The machine shut down, either because Employee #1 had hit the emergency shut-off or because the machine automatically stopped after he was pulled in. The coworker put the winder in reverse to release Employee #1. He was transported to St. John's Mercy Medical Center, where he was pronounced dead at approximately 2:43 p.m.
