Employee dies after a fall from a forklift

On February 26, 2002, Employee #1 and a coworker were working in a retail store unloading a pickup bed toolbox from an 11-foot-high shelf. The boxes were in cardboard and were stacked three high on the shelf. A standup forklift was being used with a 40-in. by 48-in. wooden pallet slid over the forks. Employee #1 stood on the pallet while the other employee, a forklift operator, hoisted him up. Upon reaching the toolboxes, Employee #1 slid the top one off when it slid and hit the pallet and his leg. This knocked him off the pallet. During the fall, Employee #1 attempted to grab something to prevent his fall. He grabbed a toolbox that eventually fell on him after he hit the concrete floor. Emergency medical services transported Employee #1 to the hospital, where he later died. At the time of the accident, no PPE was in use, and there were no guardrails on the pallet.
