Investigation Summary
Employee #1 was operating a single-opening, down-stroking press manufactured by the Dieffenbacher Company to make presswood pallets. A co-worker noticed smoke at the Number 7 press and found the guard in a raised position and Employee #1 caught in the closed carriage frame. Employee #1 had reached into an opening of the carriage frame while it was operating in the continuous mode and was pulled into the press. Employee #1 suffered injuries to the back of his neck and was killed.
