Investigation Summary
On March 21, 2002, Employee #1 was loading an empty, 22-ft, 3,500-lb, roll-off container onto the rear section of a Clement triple-axle, two-box, roll-off trailer. A coworker noticed the container rail riding on top of the trailer rail instead of riding outside the trailer rail on rollers as designed, and attempted to warn Employee #1. The coworker witnessed Employee #1 try to run to safety as the roll-off container fell off the trailer, crushing Employee #1, and killing him.
