Employee killed when struck in the head by falling object

A worker was working on a stainless steel pipe elbow suspended on a chain hoist. The elbow was 32 inches in diameter and was being installed through a hole in the floor. The chain hoist was suspended from I beams on the ceiling. The chain hoist failed and the elbow and hoist fell through the floor striking Employee #1 in the back of the head killing him.
