Investigation Summary
At 4:00 a.m. on March 21, 2001, Employee #1 was mixing dough at the mixer. Other employees in the area had gone on break and when they returned they found Employee #1 caught up in the dough trough hoist and the discharge chute to the mixer. Employee #1 was pronounced deceased upon the arrival of the paramedics.
