Investigation Summary

Investigation Nr: 201852423
Event: 03/11/2002
Employee Crawls Under Truck and Is Killed

On March 11, 2002, a crew working for Union Milling Company was cutting down trees in a log yard. Employee #1, the truck driver for Dunaway Timber Company, had the trailer filled with logs and needed to be pushed up a hill so that he could leave the yard. The foreman told the skidder operator to push him out. Employee #1 apparently crawled under the truck to check or adjust something before the skidder operator arrived to push him. When the skidder began pushing the tractor trailer, Employee #1 was pinned to the ground and killed.
