Employee Killed When Struck By A Falling Box

At approximately 12:10 p.m. on June 21, 2001, Employee #1 was placing boxes on a pallet inside a cold storage freezer. Suddenly, a 25-pound box that was left unsecured fell from the storage rack approximately 23 feet from the ground, struck Employee #1's head, killing him. The facility where the accident occurred was primarily engaged in the cold storage of perishable goods.
