Investigation Summary
At 1:30 p.m. on June 21, 2001, Employee #1 was working for Swedge Lock Systems, Inc., a metal fencing and railing manufacturing company. Employee #1 was assisting another employee on a hydraulic power press, Chicago Model Number 265, Serial Number L-11200, on a special order of green plastic rails. Employee #1 stood at the back of the machine to ensure the rails were aligned correctly. The other employee lowered the power press when he thought he heard Employee #1 say, "lower the press", when in fact Employee #1 had said the rail was off line. Employee #1 was in the process of moving the rail when the power press came down on his hand, crushing his left middle finger and amputating approximately 0.5 inch off the tip of his left ring finger.
