Employee decapitated when struck by block and cables

On October 10, 2001, Employee #1 was part of a well servicing crew in the process of pulling sucker rods and a pump from a production well. While the traveling blocks were being lowered to pull a portion of pipe, the drum brake was applied, but the traveling blocks did not slow or stop. As Employee #1 turned and attempted to run, he was struck from behind by a block and cables, resulting in his decapitation.
