Employee killed when oil-saver assembly hits his head

Employee #1 was an employee of Production Fishing and Rental Tools, Incorporated. He was standing on the floor of a work-over rig owned and operated by Pool Company Texas, Limited. Employees of the Pool crew were performing a swabbing operation on a well when the sand cable broke. A portion of the cable was left hanging down the well. The crew began to lower the lubricator/oil-saver assembly to the rig floor to remove the cable. Employee #1 was standing by to remove the hoses from his reverse pumping unit and to facilitate removal of the cable. As the lubricator was lowered, a chain securing the assembly to the winch line hook slipped and the assembly fell. The assembly struck a pipe "hammer union," causing the upper half of the assembly to separate and strike Employee #1 on the head. He was killed instantly.
