Investigation Summary
On August 13, 2001, Employee #1 was operating a Toledo full-revolution power press with two hand grips. Employee #1 placed his hands in the die feed to remove scrap. As he removed scrap, the clutch rod broke. This caused the ram to continue running and ultimately crushing Employee #1's small finger on his left hand. Employee #1 received lacerations and was not hospitalized.
