Investigation Summary
On March 12, 2002, Employee #1 was feeding plastic sheet from a roll into the nip point of a machine. The guard opening was large enough to allow her hand to touch the rolls. The rolls pulled her hand into the machine's grinding action, amputating her hand at the wrist. Employee #1 was hospitalized for her injuries.
