Investigation Summary

Investigation Nr: 200621290
Event: 12/26/2000
Employee killed when caught in between two vehicles

On December 26, 2000, Employee #1 and other employees were installing a wrecker bed, when the owner decided to backup the wrecker. The owner reversed the direction of the wrecker into a parked semi-trailer to push the wrecker bed further onto the frame. Not check whether anyone was behind the wrecker, the owner did not see Employee #1 off to the side and struck him. He was crushed between the wrecker and the stationary semi-trailer and suffered fatal injuries.
