Investigation Summary

Investigation Nr: 201351988
Event: 07/16/2002
Employee Is Killed after Being Run over by Truck

On July 16, 2002, an employee was working as a superintendent at the job site. A tractor trailer arrived at the site to deliver a crane. The driver parked the truck on a public roadway adjacent to the construction site. The employee informed the driver that the driver had to reverse the truck so to be able to enter the job site at the construction traffic entrance. The employee and a coworker of another contractor acted as flag men to direct the driver, and to flag or control traffic. The employee and the coworker did not wear red or orange warning garments or vests while flagging. The employee was walking backwards, behind and facing the truck. He was near one of the rear tires. The truck was moving approximately four to five miles per hour in reverse. The employee turned so that his back was to the truck. The rear tire of the truck ran over the employee's foot. This knocked him to the ground, and he was crushed by the truck. He was taken to a local hospital where he later died from his injuries.
