Employee Killed When Run Over By Truck Trailer

Employee #1, a checker who recorded information on trucks that were hauling trailers from a marine terminal facility, was killed when a truck moved forward and the back tires of the trailer ran over her. The operation is called the trailer interchange receipt process, and takes place at an office trailer which has two traffic lanes on each side of it for trucks to use to enter and exit the marine terminal. Employee #1 was working on the incoming side. There is a thick yellow line marked on the ground that separates the two incoming lanes. She was standing on the line. A truck entered one of the incoming lanes. It was hauling a bulk liquid transfer trailer. The driver of the truck stopped a little before where he usually parked, because a security guard was throwing some material (oil dry) on some spilled oil on the lane in front of the truck. At this time, Employee #1 stepped in front of the rear tires of the trailer to get the information that she needed. Once the security guard moved from the lane in front of the truck, the truck driver pulled forward a few feet to park where he normally stopped. When the truck moved forward, the back tires of the trailer ran over Employee #1's body and head killing her.
