Investigation Summary
At approximately 5:00 p.m. on January 22, 2001, five employees of a roofing company were working on the roof of a municipal building. No guardrail systems, safety net system or personal fall arrest systems were being used. The working surface is a flat roof with an 18-in. parapet wall surrounding three sides. The roof has three sides that expose employees to a fall of approximately 32 ft to the ground, and one side that connects with the existing old roof which has a 39-in. parapet wall encircling it. Coworkers #1 through #3 were on the southwest side of the roof and were not involved in the accident so they will not be discussed. Coworker #4 and Employee #1 were on the northwest side of the roof where the accident occurred. Coworker #4 was changing mops to spread asphalt while Employee #1 was hoisting bundles of insulation onto the roof using a small gas powered Smith hoist. Coworker #4 heard a scream and looked up to see Employee #1 loosening a knot in the hoist cable when he apparently lost his balance and fell from the roof. Employee #1 died fifteen days later due to complications from blunt head trauma sustained in the fall.
