Investigation Summary
At approximately 6:25 p.m. on October 9, 2000, Employee #1 was performing elevator maintenance work. He rode on top of the elevator in order to operate it while it was in the inspection mode. It is believed that Employee #1 looked over the edge of the elevator and was struck in the back of the head by the dust protector and died.
