Employee struck by falling pipe from lift truck

At approximately 7:15 a.m. on March 22, 2002, a coworker was lifting a 24-gauge, 10-ft duct pipe with a Super Genie Lift. Coworkers had changed the position of the forks on the lift, which allowed the pipe to roll off of the lift and onto Employee #1. He suffered bruises, contusions, and abrasions.
