Employee injured loading boxes

On March 22, 2001, Employee #1 was loading corrugated boxes into a 24 straight truck at a warehouse. After dropping a pallet of flattened corrugated boxes in the truck, he was backing the truck out of the straight truck and into the warehouse. When Employee #1 reached the bridge plate, the truck rolled forward creating a gap between the truck and the bridgeplate. The forklift truck fell into the chasm, landing on the ground. Employee #1 was able to contact another employee using a two-way radio and asked for help. The emergency services personnel arrived shortly thereafter and Employee #1 was transported to the hospital where he was treated for a fracture of the forearm.
