Investigation Summary
At approximately 1:30 p.m. on June 26, 2001, Employee #1 was in the process of installing aluminum siding on a sports facility, along with three other employees. They were working from a hydraulic tower scaffold. Employee #1 was in the process of carrying back some working materials to load onto the scaffold. He stepped back onto a two-inch by twelve-inch wood plank. The crew was using the plank as a walkway from the building to the scaffold. The wood plank had been removed, and Employee #1 fell through the opening, where the two-inch by twelve-inch wood plank was previously located. Employee #1 was killed after falling approximately 55 feet to the ground.
