Investigation Summary
An employee was outside the UPS building on a break. He was standing at dock 176 facing the building. The driver of the shifter looked several times towards the dock and sounded his horn, then backed the trailer into the dock area. The employee was caught between the dock and the back of the trailer as it was being backed to the dock area. The employee died from his injuries.
