Investigation Summary
Employee #1 was part of a crew working on an irregular steel structure. The crew was trying to reinforce a fixed ladder to allow the cutting off of a vertical beam. Employee #1 was oddly poised with his right leg on the ladder rung and his left leg on the small scaffold at the far side. He was trying to tack weld the side of the ladder. He was wearing a fall arrest system but had unhooked it to reposition himself to the side. He suffered a heart attack and lost his balance, struck the frame on the way down, and died.
