Investigation Summary
Employee #1, a carpenter, was installing sheathing on a steep roof. He was using a job made ladder as a fall protection device, designed to catch onto the peak of the roof and horizontal rafters. One of the rafters had not been raised in place; consequently, there was not a horizontal member for the ladder to catch onto. The opening caused the ladder to topple over throwing Employee #1 to the ground. He fell approximately 15 ft to the snow covered ground and died.
