Employee Killed When Struck By Ball Valve

At approximately 3:30 p.m. on January 19, 2001, a drilling crew was in the process of filling production casing on an oil drilling rig. This is done by running hard line from the mud tank to the mud pump and then hard line from the mud pump to the rig floor. On the rig floor the hard line is hooked up to a stand pipe. On the stand pipe there was a 2-in. nipple, a ninety degree elbow, another 2-in. nipple and then the ball valve. On the other end of the ball valve was a 2-in. threaded male coupling that would hook up to a "Kelly" hose. The ball valve was used to stop the flow of mud when the casing was full. At the time the accident the rig crew was in the process of filling the casing. It is unclear what caused the ball valve to come apart. Employee #1 was struck in the head by the ball valve that came apart and was killed
