Employee killed in fall through floor opening

At approximately 9:00 a.m. on July 15, 2000, Employee #1 and a coworker picked up a piece a plywood that was covering a 2 ft by 3 ft floor ladder hatchway opening. Employee #1 was facing east as he and the coworker simultaneously picked up the piece of wood. As they began to walk, he stepped into the floor opening and fell 28 1/2 ft to the ground. He sustained multiple skull fractures and was killed.
