Between 7:22 p.m. on September 21, 2001, and 12:43 a.m. on September 22, 2001, Employee #1, the night security guard from the Safety and Security Services, Inc. was killed when a rod-mounted, free standing drive-in forks fell over and struck him on the chest. The accident took place at a batch plant in the construction parking area.
