Employee killed in fall when steel column topples

At approximately 1:45 p.m. on July 31, 2000, Employee #1 and three coworkers were installing a 660 lb center solid web rafter atop a 300 lb, 25 ft tall metal column. The base of the column was fastened to four newly replaced concrete anchor bolts. Employee #1 and a coworker were on an elevated work platform, tightening the bolts holding the rafter to the column. Another coworker was operating the platform lift vehicle, while the third coworker, the proprietor of the company, was operating a second lift vehicle stabilizing the column and rafter. When the owner removed the securing vehicle to retrieve another rafter, the standing column and rafter began to topple. The column pulled out the anchor bolts and the structure overturned, causing Employee #1 to fall approximately 20 ft from the work platform onto the concrete floor. He was killed. The work platform was not equipped with guardrails, nor was Employee #1 using any form of fall protection.
